Question: About 12 years ago i started experiencing joint pain and swelling and also had fluid in my knee and had it drained. I was checked for lupus then and it was negative but my joint pain has never fully went away. I'm plagued by it and my rheumatologist diagnosed it as reactive arthritis. I have had normal CBC's since then but i've been told lupus is hard to diagnose, but if i had it since 12 years ago wouldn't i probably be really sick by now??

A CBC will not detect lupus, but it can provide a clue. Anemia, leukopenia, differential, and platelet count can all be affected by lupus, but also by many other things. Lupus is diagnosed based on symptoms, history, a wide variety of lab tests, and after ruling everything else out. It is hard to diagnose. 10 million Americans have a positive ANA, but only 1.5 million have lupus. Of the 1.5 million who have lupus, 2-5% have a negative ANA. Everyone who has a positive LE prep has lupus, but not all lupus patients have a positive LE prep. There are over one hundred biomarkers that could mean lupus, but not all of them are found in every person. This is why lupus is so hard to diagnose based on labs alone. You could have mild to moderate lupus with no permanent damage and live with it all your life. Lupus is a remitting/flaring disease which means it has periods of activity and periods of relative calm. It is not progressive although if the patient has organ involvement that can progress if left untreated. Many lupus patients have no organ involvement at all.

No, not a simple CBC. There is no single test for Lupus because Lupus affects different people in different ways. The blood can be tested for certain proteins, ANAs, (antinuclear antibodies,) but other problems can cause the body to make ANAs as well. Your doctor would make a diagnosis by asking you questions regarding your symptoms in general and more specific blood testing.

no, a typical CBC would not be diagnostic for lupus. there are more specific tests for lupus that would have to be ordered. If you had a joint effusion 12 years ago, that indicates some problem with the joint. have you seen a regular orthopedic surgeon?

Blood cells originate in the bone marrow. High WBC usually indicates an infection. Anisocytosis is variation in the size of red blood cells (an indication of anemia) elevated neutrophyls indicate an acute infection (neut) Elevaten eosinophils may be due to parasitic diseases, bronchial asthma or hay fever. Eosinopenia may occur when the body is severely stressed. (eosin) If the count showed an abnormally high number of basophils, hemolytic anemia or chicken pox may be the cause. (baso) CRP is a marker of infection and inflammation. Discuss the results with ur dr, it is ur right to fully understand why he wants to order certain tests, and if u do not understand or it is not explained to u, u should make someone explain before u have the procedure preformed.

My sister is a carrier but does not exhibit any of the symptoms. She found out when she went through in-vitro pregnancy. She had to have the boys tested after they were born.
